At what altitude should supplemental oxygen be used for a prolonged flight under 14 CFR 91.211?

Answer

Crew must use supplemental oxygen for flight above 12,500 feet MSL to 14,000 feet MSL if that altitude is exceeded for more than 30 minutes, and at all times above 14,000 feet MSL. Passengers must be provided O2 above 15,000 feet.
