What causes an altimeter to read incorrectly when flying from high-pressure to low-pressure air?

Answer

'From High to Low, look out below.' Flying into lower pressure without resetting the Kollsman window causes the altimeter to read higher than actual altitude — the aircraft is lower than indicated.
